Hospice Services:

Medical Services:

Primary Care Physician: The patient's attending physician works in conjunction with the Hospice staff for the benefit of the patient.

Hospice Medical Director: As a member of the Interdisciplinary Team, this physician reviews the patient's Plan of Care and is available for consultation regarding the medical management of the patient's care.

Nursing Services:

Primary Care Nurse: A registered nurse, as a member of the Interdisciplinary Team coordinates hospice services for the patient and provides skilled nursing care. This person also teaches and supports the family regarding care of their loved one.

Personal Care Provider: This may be an R.N., L.P.N., C.N.A. or a Nursing Assistant, providing personal care for the patient as appropriate for his/her comfort.

Social Services:

Social Worker: The role of this member of the team is to support patients and care givers as they face the emotional stress associated with life-threatening illness. He/She also helps families to connect with community resources.

Pastoral Care Provider: Upon request, this person visits the patient and family to explore issues regarding the process of living and dying. It is often possible to include the patient/family's own spiritual advisor as a member of the hospice team.

Volunteer Services:

Hospice Friend: A volunteer and member of the Interdisciplinary Team who visits on a regular or intermittent basis to provide emotional support and assistance to the patient and family.

Bereavement Services:

A comprehensive program of supportive services offered to the patient's family during the first year following their loss. The program is individualized based upon the family's expressed needs.

Respite Care:

The goal is to provide temporary relief for the patient's family, or other care givers in the event of an unforeseen emergency or to relieve the caregiver or family of some of the daily demands of care for the patient.

InPatient Acute Services:

These include short periods of stay in a hospital when pain and symptoms of the terminal illness require intense management. Hospice of the Plains, Inc. maintains contracts with all area hospitals.
