Who Does Hospice of the Plains Serve?
- Persons who live in Logan, Phillips Sedgwick, Washington and Yuma Counties.
- Persons whose primary physician practices in these counties.
- Each patient must meet the admission requirements as set forth by Federal regulations.
- Services are provided regardless of race, national origin, religion, gender, sexual orientation, age, disability,
terminal diagnosis, social status or ability to pay.
